DRIVER REQUIREMENTS

• Eligible to work in the United States
• CDL Class A, From State of Residence
• Minimum: 22 years old
• Current Medical Card Within The Last 21 Months
• Within the last 10 years:
  – No Felony Convictions
• Within the last 5 years:
  – 12-17 Months Verifiable Tractor Trailer Experience, With Road-Test
  – 18 Months Verifiable Driving Experience, No Road-Test
  – NO Drug/Alcohol Offenses
  – NO Major Preventable Accidents
• Within the Last 3 years:
  – No CDL Suspension
  – 4 or Fewer Moving Violations
  – NO Reckless Offenses, Never In A CMV
  – Good Employment History
  – Good Safety Record

TRUCK SIGN ON REQUIREMENTS

• Signed Lease Agreement With B&W Cartage/TVM
• 12 year old or Newer Trucks Only
• Truck Title in Owners Name
• DOT Annual Inspection Within the Last 45 Days Done At A “Preferred Vendor” (Please Call For A List Of Vendors In Your Area)
• Need Inspector Qualification Certificate From Shop
• Non-Trucking Liability Insurance For $1,000,000
• Current Registration For 80,000lbs. Unless Using Our Plate
• Completed W-9
• EIN Letter From the IRS With Your Company Name
• Fifth Wheel No Higher Than 47″
• Signed Satellite Agreement
• Signed IFTA Permit Request
• $2000.00 Escrow Account, Will Be Deducted From Settlements For 20 Weeks
